Sapphire ore and Altar of Refillment for it + other stuff

Heya!

I made a new cave for the underwater map in Piratetown for a new altar. It restores Sapphire nodes. The colors of the cave and the colors are new!

I also added some Sapphire nodes you can mine in the underwater map. You need a Sapphire mining pick to mine them, which you can get from the smith in town.

Here's the icon for Sapphires and the Sapphire Mining Pick.

Also a new icon for the Water Dungeon Portstone.
